REBATE FORM

How to receive your rebate:

1. Purchase a new qualified clothes washer from January 1, 2008,
through December 31, 2008, and install it in the service area of one of the participating utilities.

2. Complete the entire front portion of the rebate application form
and sign it.

3. Make a copy of the completed rebate application and receipt for your records.

4. Mail the completed copy of the rebate application and a copy of      your itemized invoice or sales receipt within 90 days of purchase
     of clothes washer.

5. Only one rebate per household. Not eligible if you have received
    a WashWise rebate for this home after January 1, 2001.

All rebate requests must be received no later than March 31, 2009.
